EPG Icons duplicating and also not showing on ALL my channels?? Why is this happening and how can I fix?

Is this mytvonline? If it is, then I'm pretty certain the channel logos can only be changed by the provider at their end.

It could be your provider, also try to delete cache, restart your box and see what happens, if Not , you can also try to delete that url entry in your settings and re enter your info.
